Below is a description of what we want developed!
We can afford $200, if necessary - we can lift the token abit.

Specs for bid:

On the existing IAHP website, http://hydroseeding.org/ we need an online survey that can be filled out. These surveys will have a section of information that we want to keep private, but it stays with the survey so I can see it. The balance of the survey will become part of a searchable data base that anyone can access via the net.

* The content of the surveys must be editable by myself.

* The questions of the survey must be editable by myself, so I can add or subtract questions as needed.

* A sample of the survey is attached.

* The survey data base must be searchable by the viewers, probably by location as the most common one.


I want to be able to add new surveys/ and pages of data.

This what I had in mind, I don't know if there is a better way, if there is, please make suggestions:

* someone fills out the online survey at the IAHP website

* I log in and see the newly completed surveys. I edit them if needed

* I then load them into the data base for online viewing


The home page for the surveys would include links for filling in the survey, links to the various data bases that we will be adding in the future, and some method of accessing the search engine for the data bases.
